I've found that to fix small changes like this, rapid fire in large quantities is the key.
Yes it's still important to practice like you play (sometimes) and line up every putt... but rapid fire helped me dial in control of the angles.

My form was bad and I've been working hard on it for about a year now. I've got it to be much better now however I'm not sure how to level up from here.
I used to throw 300' with bad form and now I throw 300' with much better form.
But my accuracy is better so at least there's that.
I can hit...
... when people spit loogies on the ground all around the practice basket...
it's usually the same guys that piss in the bushes behind the baskets on the course.
And with that said ... it's gross to see some players rub their hands in the dirt... and then lick their fingertips. ;-P
