Form Critique, Please.

So I have been playing almost 4 years, I am fairly accurate with my drives but cap out around 250'. I also have trouble getting some higher speed discs to fly how they are supposed to(I'm talking talks, not Destroyers or Boss's. I have started discing down to practice form but I don't really know what I am doing wrong or could be doing differently. Attached is a video I took earlier today, 3 different angles, 2 throws from each angle, all in slow motion. Any help would be greatly appreciated, thanks in advance!
