Form Advice/Critique

I've been playing for a while now and I've hit a wall with my backhand form. My accuracy is improving very slowly but my max distance has been the same for quite some time. I can consistently hit 320 and I can get some to 350 on a good day. I typically don't throw anything faster than a 9 speed and my max distance shots are flat releases with slightly understable discs. They turn a little at high speed and fade back at the end of the flight (ie valkyries+escapes). I'm not sure what to work on anymore and I figured this forum would be a good place to start. My current goal is to be able to hit 360 consistently by the end of the year.

I'm mostly looking for a few key things to work on right now (whatever is most important). I don't want to change too many things at once. I'd rather make small adjustments over time. Any critique is much appreciated though.

I would also appreciate any recommendations of people to model my form off of that have a similar build. I'm 5'7" with a thin build.

I think the Jarvises were about your build. No one finer to emulate if you want to develop raw power throwing a disc. SW just posted this old vid but he's probably a lot better to tell you how to get there.
 
I'm 5'7", Emerson Keith is 5'6".

You are trying to rotate open too much and looking at the target instead of the disc. Pay attention to how you swing the disc, not the target.
