Do my friends count? I've played a few rounds by myself and while enjoyable, just not the same. Other than that, knee brace.
 
My phone to track my blood sugar, the handheld device that controls delivery from my insulin pod, and sugary food (coke, dried fruit) to raise my blood sugar as needed. Also a towel to keep my insulin pod adhesion dry in the Baltimore humidity. I never pack all that light, which is a bummer.
 
My stool is very important. I've been using an Innova Caddy Seat for a long time. Just replaced one I'd been using for 4-5 years. Lightweight & fits my frame. Couldn't survive a tournament without one.

Hats are crucial for me since I'm bald. Sunburns on top of my head are unacceptable.
 
Beer?

Really water or gatorade is the only thing I have to have on course other then disc.

Uh, my bag. It's a pain in the butt to carry ~17 discs around without one so I'd say it's pretty essential.
 
Bag, towel, disc gator/other retrieval device, in that order
 
For me, it's a hat.

I tried playing with sunglasses, but they somehow mess up depth perception.

No matter where I play, or how casual the round, I always have a baseball-type hat on.
 
For the grizzled tournament veteran where two long 18s in a day is pretty normal, you MUST have some diaper rash cream in your bag. Lube up before round one if you wanna make it through day.
